A narrative is constructed to elicit a particular response from its audience. In the form of a written story, authors use specific narrative strategies to position the ‘ideal reader’ to attain the intended understanding of the meanings in the text. Oliver Sacks’ short story The Man Who Mistook His Wife for a Hat is an unusual short story because it does not display conventional plot development; the story does not contain conflict or resolution of conflict. The genre of the story is also difficult to define because it reads as an autobiographical account of an experience Sacks had with a patient while working as a neurologist. Analysing the elements and conventions of The Man Who Mistook His Wife for a Hat makes it more obvious that the story is indeed a construction, that elements have been chosen and omitted to create meaning: â€Å"... texts offer a selection which conforms to certain limited beliefs a nd values.† (Moon) The selections in the text are also organised into a plot. Reasons why he named it that way was because he mostly focused on young adults who had dementia. Eugene Bleuler, a Swiss psychiatrist, coined the word schizophrenia in the early 1900’s. He later changed the name as the name Dr. Kraepelin gave the disease was the incorrect term for the disorder. The word schizophrenia is derived from the Greek root words Schizo which means split and phrene which is mind. In this snap shot, we see many unfortunate things. Shakespeare gives us the idea that many people try to circumvent what the future holds, such as unfortunate things, by being superstitious. Superstition seems to play a role in the basic daily life of most Roman citizens. For instance, the setting of the first scene is based upon superstition, the Feast of Lupercal. This feast is in honor of the god Pan, the queen of fertility. During this time, infertile females are supposed to be able to procreate, and fertile ones are supposed to be able to bear more. It is also a supposed time of sexual glorification and happiness. Other scenes depict how throughout Rome, roaming the streets are mysterious sooth-sayers, who are supposedly given the power to predict the future. Dictating what is to come through terse tidbits, these people may also be looked upon as superstitiou s. In the opening scene, one sooth-sayer, old in his years, warns Caesar to Beware the Ides of March, an admonition of Caesars impending death. Although sooth-sayers are looked upon by many as insane out of touch lower classmen, a good deal of them, obviously including the sayer Caesar encountered, are indeed right on the mark. Since they lack any formal office or shop, and they predict forthcomings without fee, one can see quite easily why citizens would distrust their predictions. Superstition, in general elements such as the Feast of Lupercal, as well as on a personal level such as with the sooth-sayers, is an important factor in determining the events and the outcome of Julius Caesar, a significant force throughout the entire course of the play. Before the play fully unravels, we see a few of signs of Caesars tragic end. Aside from the sooth-sayers warning, we also see another sign during Caesars visit with the Augerers, the latter day psychics. They find No heart in the beast, which they interpret as advice to Caesar that he should remain at home. Ceasar brushes it off and thinks of it as a rebuke from the gods, meaning that he is a coward if he does not go out, and so he dismisses the wise advice as hearsay. However, the next morning, his wife Calphurnia wakes up frightened due to a horrible nightmare. She tells Caesar of a battle breaking out in the heart of Rome, Which drizzled blood upon the Capitol, with Caesar painfully dying, such that The heavens themselves blaze forth the death of princes. Although Caesar realizes Calphurnia is truly concerned about his well-being, he seeks another interpretation, coming to the conclusion that the person who imagines the dream may not be the wisest one to interpret its meaning. Lat er Caesar tells his faithful companion Decius about it, and he interprets it quite the contrary, That it was a vision fair and fortunate, and indeed, today is an ideal day to go out, since this is the day To give a crown to mighty Caesar. Perhaps Decius is implying here that today is a day where much appreciation and appraisal will be given to Caesar, surely not the endangerment of his well-being as Calphurnia interprets it. Caesar predictably agrees with him, as most citizens enjoy believing the more positive of two interpretations. After Caesars assasination at the hand of Brutus, Cassius, and the rest of the conspirators, Brutus and Cassius are chased into the country side, where we see a few superstitious signs of their forthcoming painful death in battle. In a dream, Brutus sees Caesars ghost, interpreted as an omen of his defeat. He also looks upon the ensign, and instead of the usual stock of eagles, ravens and kites replace them, construed as another sign of their loss at Phillipi. Not surprisingly, Caesars death is avenged in the end, with the two of the conspirators double suicide. As superstition is inter-twined within the basis of the entire play, we can reasonably conclude that it is because of this irrational belief of why certain events occur and how to avoid them, that Caesar is retired and eventually avenged. Henry J. Raymond

Henry J. Raymond Henry J. Raymond, political activist and journalist, founded the New York Times in 1851 and served as its dominant editorial voice for nearly two decades. When Raymond launched the Times, New York City was already home to thriving newspapers edited by prominent editors such as Horace Greeley and James Gordon Bennett. But the 31-year-old Raymond believed he could provide the public with something new, a newspaper devoted to honest and reliable coverage without overt political crusading. Despite Raymonds deliberately moderate stance as a journalist, he was always quite active in politics. He was prominent in Whig Party affairs until the mid-1850s, when he became an early supporter of the new anti-slavery Republican Party. Raymond and the New York Times helped bring Abraham Lincoln to national prominence after his February 1860 speech at Cooper Union, and the newspaper supported Lincoln and the Union cause throughout the Civil War. Following the Civil War, Raymond, who had been the chairman of the National Republican Party, served in the House of Representatives. He was involved in a number of controversies over Reconstruction policy and his time in Congress was extremely difficult. Habitually afflicted by overwork, Raymond died of a cerebral hemorrhage at the age  of 49. His legacy was the creation of the New York Times and what amounted to a new style of journalism focused on the honest presentation of both sides of critical issues. Early Life Henry Jarvis Raymond was born in Lima, New York, on January 24, 1820. His family owned a prosperous farm and young Henry received a good childhood education. He graduated from the University of Vermont in 1840, though not after becoming dangerously ill from overwork.   While in college he began to contribute  essays to a magazine edited by Horace Greeley. And after college he secured a job working for Greeley at his new newspaper, the New York Tribune. Raymond took to city journalism, and became indoctrinated with the idea that newspapers should perform a social service. Raymond befriended a young man in the Tribunes business office, George Jones, and the two began to think about forming their own newspaper. The idea was put on hold while Jones went to work for a bank in Albany, New York,  and Raymonds career took him to other newspapers and deepening involvement with Whig Party politics. In 1849, while working for a New York City newspaper, the Courier and Examiner, Raymond  was elected to the New York State legislature. He  was soon elected speaker of the assembly, but was determined to launch his own newspaper. In early 1851 Raymond was conversing with his friend George Jones in Albany, and they finally decided to start  their own newspaper. Founding of the New York Times With some investors from Albany and New York City, Jones and Raymond set about finding an office, purchasing a new Hoe printing press, and recruiting staff. And on September 18, 1851 the first edition appeared. On page two of the first issue Raymond issued a lengthy statement of purpose under the headline A Word About Ourselves. He explained that the paper was priced at one cent so as to obtain a large circulation and corresponding influence. He also took issue with speculation and gossip about the new paper which had circulated throughout the summer of 1851. He mentioned that the Times was rumored to be supporting several different, and contradictory, candidates. Raymond spoke eloquently about how the new paper would address issues, and he seemed to be making reference to the two dominant temperamental editors of the day, Greeley of the New York Tribune and Bennett of the New York Herald: We do not mean to write as if we were in a passion, unless that shall really be the case; and we shall make it a point to get into a passion as rarely as possible. There are very few things in this world which it is worthwhile to get angry about; and they are just the things that anger will not improve. In controversies with other journals, with individuals, or with parties, we shall engage only when, in our opinion, some important public interest can be promoted thereby; and even then, we shall endeavor to rely more upon fair argument than upon misrepresentation or abusive language. The new newspaper was successful, but its first years were difficult. Its hard to imagine the New York Tijmes as the scrappy upstart, but thats what it was as compared to Greeleys Tribune or Bennetts Herald. An incident from the early years of the Times demonstrates the competition among New York City newspapers at the time. When the steamship Arctic sank in September 1854, James Gordon Bennett arranged to have an interview with a survivor. Editors at the Times thought it unfair that Bennett and the Herald would have an exclusive interview, as the newspapers tended to cooperate in such matters. So the Times managed to get the earliest copies of the Heralds interview and set it in type and rushed their version out to the street first. By 1854 standards, the New York Times had essentially hacked the more established Herald. The antagonism between Bennett and Raymond percolated for years. In a move that would surprise those familiar with the modern New York Times, the newspaper published a mean-spirited ethnic caricature of Bennett in December 1861. The front-page cartoon depicted Bennett, who had been born in Scotland, as a devil playing a bagpipe. Talented Journalist Though Raymond was only 31 when he began editing the New York Times, he was already an accomplished journalist known for solid reporting skills and an astounding ability to not only write well but write very fast. Many stories were told about Raymonds ability to write quickly in longhand, immediately handing the pages to compositors who would set his words into type. A famous example was when the politician and great orator Daniel Webster died in October 1852. On October 25, 1852, the New York Times published a lengthy biography of Webster running to 26 columns. A  friend and colleague of Raymonds later recalled that Raymond had written 16 columns of it himself. He essentially wrote three complete pages of a daily newspaper in a few hours, between the time the news arrived by telegraph and the time the type had to go to press. Besides being an inordinately talented writer, Raymond loved the competition of city journalism. He guided the Times when they battled to be first on stories, such as when the steamship Arctic sank in September 1854 and all the papers were scrambling to get the news. Support for Lincoln In the early 1850s Raymond, like many others, gravitated to the new Republican Party as the Whig Party essentially dissolved. And when Abraham Lincoln began to rise to prominence in Republican circles, Raymond recognized him as having presidential potential. At the 1860 Republican convention, Raymond supported the candidacy of fellow New Yorker William Seward. But once Lincoln was nominated Raymond, and the New York Times, supported him. In 1864 Raymond was very active at the Republican National Convention at which Lincoln was renominated and Andrew Johnson added to the ticket. During that summer Raymond wrote to Lincoln expressing his fear that Lincoln would lose in November. But with military victories in the fall, Lincoln won a second term. Lincolns second term, of course, only lasted six weeks. Raymond, who had been elected to Congress, found himself generally at odds with the more radical members of his own party, including Thaddeus Stevens. Raymonds time in Congress was generally disastrous. It was often observed that his success in journalism did not extend to politics, and he would have been better off to stay out of politics entirely. The Republican Party did not renominate Raymond to run for Congress in 1868. And by that time he was exhausted from the constant internal warfare in the party.   On the morning of Friday, June 18, 1869, Raymond died, of an apparent cerebral hemorrhage, at his home in Greenwich Village.  The next days New York Times was published with thick black mourning borders between the columns on page one. The newspapers story announcing his death began: It is our sad duty to announce the death of Mr. Henry J. Raymond, the founder and editor of the Times, who died suddenly at his residence yesterday morning of an attack of apoplexy. The intelligence of this painful event, which has robbed American journalism of one of its more eminent supporters, and deprived the nation of a patriotic statesman, whose wise and moderate counsels can ill be spared at the present juncture of affairs, will be received with deep sorrow throughout the country, not alone by those who enjoyed his personal friendship, and shared his political convictions, but by those also who knew him only as a journalist and public man. His death will be felt as a national loss. Legacy of Henry J. Raymond Following the death of Raymond, the New York Times endured. And the ideas advanced by Raymond, that newspapers should report both sides of an issue and show moderation, eventually became standard in American journalism. Raymond was often criticized for not being able to make up his mind about about an issue, unlike his competitors Greeley and Bennett. He addressed that quirk of his own personality directly: If those of my friends who call me a waverer  could only know how impossible it is for me to see but one aspect of a question, or to espouse but one side of a cause, they would pity rather than condemn me; and however much I may wish myself differently constituted, yet I cannot unmake the original structure of my mind. His death at such a young age came as a shock to New York City and especially its journalistic community. The following day the main competitors of the New York Times, Greeleys Tribune and Bennetts Herald, printed heartfelt  tributes to Raymond.

Thus, it was a manipulative question forcing the students to choose between argument or deception. In that sense, it was not an open-ended question exploring the possible reasons of the deceptive behaviors. Accordingly, the research actually found what it set out to find, the students lied in order to avoid argument; while it did not leave any other option to choose as their motive. The major gap of the study is also pointed out by the researcher herself, she noted that more research is needed i ncluding the samples of adults from different cultures. However, her sample must have been more varied in terms of the ages of the participants. It is even doubtful whether the university students are capable of handling truly intimate, adult and mature relations; since, they tended to date casually. Young people might have preferred to deceive in order to argue, since most of them are neither mature nor committed enough to develop conflict-resolution mechanisms as much as the older people. In fact, the title of the article should have been â€Å"Deception in Intimate relationships of the Australian Students†. Gerald R. Miller (1986) studied how several differences between impersonal and personal relationships may affect deceptive behavior in â€Å"Invited Article Fudging with Friends and Lying to Lovers: Deceptive Communication in Personal Relationships†. This case report highlights a commonly occurring and often reported problem of Paresthesia involving the Mental Nerve during endodontic treatment. Most of the previous case reports have used radiographs for the diagnosis and management of these cases. Since the radiographic image is a 2 dimensional image it has its limitation while a 3 dimensional imaging modality like a cone beam computed tomography (CBCT) provides the clinician with additional information which can be critical in the management of cases. The exact 3 dimensional location of the mental foramen in relation to the apex of the concerned tooth ca n be confirmed using CBCT scans, helping make more informed diagnosis and treatment plans. This case was successfully managed using CBCT images for guidance during the root canal treatment. INTRODUCTION Parasthesia is defined as a sensory disturbance with clinical manifestations such as burning, prickling, tingling, numbness, itching or any deviation from normal sensation1. Paresthesia of the Inferior Alveolar Nerve and its branches can occur during various dental Procedures like Local Anesthetic Injections, Third Molar Surgery, Orthognathic Surgery, Ablative Surgery, Implants, and Endodontics2, 3. Damage to vital structures in the head and neck area during dental treatment is always a vexing prospect for the clinician. During Endodontic Treatment precautions have to be taken against this, as there is a possibility of injury to a vital structure with the instruments or chemicals being used and also a chance of depositing infected material with their byproducts from the tooth into these areas causing a subsequent immunological response from the body. Today we are able to use 3 Dimensional imaging to locate and respond to such situations much more precisely than ever before4,5.This case highlights a commonly occurring and often reported problem of Paresthesia involving the Mental Nerve during endodontic treatment6.7 which was managed using a CBCT to offer the patient a predictable treatment. Most of the previous case reports have used OPG’S and Intra oral periapical radiographs for the diagnosis and management of these cases. Since these are 2 dimensional imaging technique s they have their limitations. Cone Beam Computed Tomography (CBCT) is 3 dimensional imaging modality which provides the clinician with additional information which can prove to be critical in the management of such cases. CASE REPORT A 32 Year old female patient was referred to the Department of Conservative Dentistry with the chief complaint of a dislodged temporary restoration in a tooth undergoing endodontic treatment at a private dental clinic. Patient gave a history of Root canal treatment having been started 2 weeks back. Clinical Examination revealed an exposed access opening and extreme tenderness to percussion. Radiographic evaluation confirmed the history presented by the patient (Fig 1). A differential diagnosis of Periapical granuloma/ Periapical Abscess was recorded and the patient was advised to complete the Root canal treatment. In the first appointment the access opening was refined under local anaesthesia and the pulp tissue remnants were extirpated using a barbed broach followed by temporistion.The patient was recalled after 4 days for Root canal instrumentation. In the second appointment local anaesthesia was administered and working length determination was done using an apex locator (Propex 2,Dentsply) followed by confirmation with a radiograph (Fig 1). The working length was kept at 0.5mm short of the radiographic length of the tooth and the root canal instrumentation was carried out. Saline and Sodium Hypochlorite were used to copiously irrigate the root canal space and instrumentation was carried out with the Protaper Rotary system. The root canal was dried and the tooth was temporized for obturation in the following visit. On the next visit 7 days later the patient complained of Parasthesia of the lower lip from the date of the previous visit. On examination there was Parasthesia of the left half o f the lower lip and its distribution was consistent with the supply of the mental nerve (Fig 2). A close examination of the IOPA revealed the presence of the mental foramen directly below the apex of the treated premolar indicating possible deleterious effects of the previous procedure causing and periapical inflammation and subsequent damage to the mental nerve. A CBCT was taken to confirm the exact location of the mental foramen. The CBCT images confirmed the extreme close nature of the mental foramen to the apex of the treated premolar (Fig 3, 4). The distance between the apex of the premolar to the mental foramen was measured to be 0.4mm by using the CBCT software. Taking this into account the working length was reset at 1.5 mm short of the radiographic apex. The tooth was then instrumented to the new working length and the canals were irrigated with saline and sodium hypochlorite followed by drying using paper points. The patient was advised that the Parasthesia will gradually disappear and that the lip will regain normal sensation and was scheduled weekly recall appointments. The Parasthesia was still present at the one week recall appointment until the 6th week after which gradual normalization was observed. At end of two months the Parasthesia had completely disappeared and normal sensation had returned. Endodontic treatment was resumed keeping the new working length. The instrumentation was completed and the tooth was obturated using F3 (Protaper Dentsply) size gutta percha for apical fill and Obtura 2 with System B for back fill using continuous wave of condensation techniqueA one week follow up was done to confirm the absence of any recurrence of the Parasthesia following the obturation. Finally the tooth was restored with a No.1 size fiber post (Angelus Brazil) and crown was placed (Fig 5). DISCUSSION Dental radiographic evaluation is a fundamental tool for endodontic diagnosis. Conventional Intraoral Periapical radiographs are routinely employed during endodontic diagnosis to examine the tooth, identify the pathology and plan the treatment. However, a conventional radiograph is a two dimensional image of a three dimensional object and consequently has limitations. Numerous prior studies have demonstrated the effective use of CBCT in the assessment of complex endodontic cases .In cases where location of a vital structure such as the mental foramen or the Mandibular canal are concerned the 3 Dimensional imaging capabilities of a CBCT are invaluable. With CBCT and its advanced 3 D reconstruction software, it is possible precisely orient the teeth under treatment with the adjacent anatomic structures across a multitude of planes to get measurable values of distance, which help with the consequent predictable treatment plan. In situations of close proximity of a nerve to a tooth which is being endodontically treated various possibilities of damage to the nerve exist. Mechanical compression of the nerve, Damage to the nerve due to over instrumentation, Extrusion of necrotic debris and toxic metabolites from the root canal space, overfill or the passage of various endodontic materials (root canal irrigants, sealers, and paraformaldehyde containing pastes) into the vicinity of the nerve or its branches. In the present case the most probable cause of the Parasthesia could have been an acute exacerbation of the Periapical infection due to Extrusion of the necrotic debris from the root canal space into the mental foramen space and/or inadvertent direct mechanical compression of the nerve because of over instrumentation of the tooth during working length determination Direct peripheral nerve injury has been previously classified into three basic types: Neurapraxia, Axonotmesis and Neurotmesis 8. Neurapraxia occurs due to a slight compression of the nerve trunk resulting in a temporary conduction block. Neurapraxia of the inferior alveolar nerve or mental nerve will usually manifest as a Paresthesia or Dysaesthesia of the lip and chin region 9. Axonotmesis refers to the actual degeneration of the afferent fibers as a result of internal/external irritation resulting in anesthesia 10. Neurotmesis is the complete severing of the nerve trunk, resulting in permanent Paresthesia which can only be corrected by microsurgery and has a more guarded prognosis (8-10). The most likely form of injury in the present case seems to be Neurapraxia due to either periapical infection or direct injury by over-instrumentation/inadvertent passage of the root canal irrigant or both. The tooth responded well to conservative treatment, and upon completion of the debridement and disinfection of the root canal, the symptoms of periapical infection subsided and Paresthesia started to diminish. CONCLUSIONS The lower mandibular premolars very frequently are in close approximation to the Mental foramen and the Mandibular canal. The pre-operative radiograph does provide us with a 2 dimensional image but better more accurate location can be obtained using the CBCT whenever possible. Finally the best method to prevent any damage to the Mental Nerve is to locate it.
